A lot of what makes care work well happens between appointments — the follow-up that actually happens, the lab result someone reviews and calls you about, the medication list that stays accurate.
We review your medications regularly to check for interactions, duplications and prescriptions that are no longer needed. We order and coordinate labs and diagnostic testing, complete preoperative medical evaluations when you are scheduled for surgery, and refer you to specialists when that is genuinely the right step.
When you are referred out, we stay in the loop. Care coordination is our job, not yours.
